    • Original SSC Certificate 
    • Original FSC / Equivalence Certificate 
    • Original Character Certificate 
    • Original Domicile Certificate
    • MDCAT Result Card 
    • Copy of CNIC of Candidate 
    • Copy of CNIC of Father 
    • At least 12 PHOTOGRAPHs of a candidate with sky blue background
    • Hafiz-e-Quran Certificate (if applicable). 
    • ATTESTED Copy of SSC Certificate, HSSC/ Fsc Certificate 
    • Surety Bond on Stamp Paper worth Rs. Three (03) million. 
    • Undertaking on Stamp Paper
    • Medical certificate 
    • Vaccination Certificate for Hepatitis B, Tetanus, Fever. 
    • Medical College Admissions Call Letter.
    • ATTESTED SETS of Photocopies of All Documents mentioned above
