Documents Required at Medical Colleges MBBS & BDS Admissions. New First-year MBBS BDS admissions in UHS Lahore Medical and Dental Colleges are required to submit the set of academic and personal documents at the time of admissions or orientation day in the selected Medical colleges.

List of Important Documents Required at Medical Colleges MBBS & BDS Admissions

  • Original SSC Certificate or Matriculation Certificate.
  • Original FSC / Equivalence Certificate
  • NOC/Migration Certificate (Other than Punjab boards if applicable)
  • Original Provisional and Character Certificate
  • Original Domicile Certificate of canditate
  • PMC MDCAT Entry Test Marks Sheet.
  • CNIC (01 Copy each) of Candidate and Father or Guardian.
  • Hafiz-e-Quran Certificate (if applicable).
  • Two (02) two Copies of the FSC Certificate for Verification
  • For UHS Registration One set of ATTESTED Copied (SSC Certificate, FSC Certificate & 2 Photograph)
  • For PMDC Registration One set of ATTESTED Copied (SSC Certificate, FSC Certificate & 3 Photograph)
  • ATTESTED 3 SETS of Photocopies of All Documents mentioned above with 12 PHOTOGRAPHS of Student ( Six 06 ID CARD size and 06 PASSPORT SIZE photographs with sky blue background)
  • You will have to submit “Receipt of scheduled fee deposit in the respective & concerned Board of Intermediate & Secondary Education” for verification of documents
  • Surety Bond on Stamp Paper Duly Attested worth Rs. Three (03) million.
  • Undertaking on Stamp Paper Duly Attested by First Class Magistrate.
  • Medical certificate
  • Vaccination Certificate for Hepatitis B, Tetanus, Innoculation against entire Group of Fever.
  • Medical College Admissions Call Letter.

Fee Instructions
After submission of required documents, students have to deposit Medical College due including tuition and admissions fees with Hostel dues if they are interested in having an accommodation facility provided by the college. The institution is not bound to provide hostel accommodation
The candidate must produce a set of certificates of vaccinations as per university policy and will be examined by a board of doctors.

Important Instruction

    I have got the medical certificate as you have uploaded.(without any inoculation. the hospital administration wasn't inoculating any medical student coming there)
    *Covid vaccination certificate is obtained from Nadra
    Is there the same rule for certificate of tetanus and hepatitis B??*
    Or the medical certificate is enough?
